Overgrowth of a Costochondral Graft Hypertelorism due to Midline Facial Cleft Unilateral Cleft Lip and Palate Craniosynostosis and Hipertelorism
Preoperative facial and intraoral views of a newborn patient with severe bilateral facial clefts, and amophthalmos or abscense of the right eyeball. The patient has undergone several reconstructive and dentofacial orthopedic procedures. Her right eyeball has been prosthetically replaced. Progress views at age 5. This patient is under treatment at the San Jorge Children's Hospital Craniofacial Foundation, San Juan, Puerto Rico. Dr. Polo is the team's staff orthodontist. Preoperative frontal view of 13-year-old patient with overgrowth of the left hemimandibular costochondral graft reconstruction performed for ankylosis of the left temporomandibular joint at an early age. Postoperative frontal view 6 months after partial resection and reconstruction of the left costochondral graft, removal of the left buccal fat pad, and a right ascending ramus sagittal-split osteotomy. Preoperative submental vertex view showing asymmetry of the mandible. Postoperative submental vertex view 6 months after first stage mandibular reconstruction before orthodontic correction. Craniofacial surgery corrections of this patient performed at the International Craniofacial Institute, Dallas, Texas. The patient displays similar conditions to individuals orthodontically treated by Dr. Mario Polo in collaboration with the International Craniofacial Institute. Photos used by permission of Dr. Kenneth E. Salyer. Preoperative frontal view of a 15-year-old patient with a midfacial cleft and hypertelorism, a bifid nose, and a deformed maxilla. Postoperative frontal view 3 years after surgical correction. Preoperative submental vertex view shows lack of nasal projection of the bifid nose. Postoperative submental vertex of the same patient after reconstruction of the nose and lip. Kenneth E. Salyer. Preoperative frontal view of 5-year-old patient with Pfeiffer's syndrome. Postoperative frontal view after Lefort III facial advancement with fronto-cranial remodeling. Pre and postoperative lateral views. Kenneth E. Salyer. Preoperative frontal view of 3-year-old patient with Crouzon's Syndrome with decreased vision resulting from increased intracranial pressure. Postoperative frontal view after cranial vault expansion and Lefort III advancement. Pre and postoperative lateral views. Kenneth E. Salyer. Preoperative frontal view of 8-year-old patient after first cranial vault remodeling at 3 months of age. Postoperative frontal view 2 years after secondary cranial vault remodeling and Lefort III advancement. Preoperative lateral view shows facial and forehead retrusion and exorbitism as well as evidence of increased intracranial pressure. Postoperative lateral view 2 years after reconstruction. Kenneth E. Salyer. Preoperative frontal view of 16-year-old patient with Treacher Collins Syndrome. No previous correction had been performed. Postoperative lateral view 1 year after orthognathic surgery. Pre and postoperative lateral views of same patient. Kenneth E. Salyer. The incidence of this condition is approximately one of every 1,500 births, depending on race and ethnicity and is more prominent in males than females (60 to 80% males). This condition is the most common craniofacial birth defect. The rehabilitation of patients with cleft lip and palate under the care of a multidisciplinary team approach - which includes the orthodontist as one of the main members - attains highly satisfactory and pleasing results.
